Here is a ski rental form I have built with jQuery. It still has a couple of bugs, but I stopped development on it when I was told the client wanted only a super simple form with some validation. But here is the link to my jQuery Masked & Validated Form.

I did this website for my friend, and brother, Ben King. He is the worship leader for the Aiteo rock band. It took 3 months (without internet) and I learned tons about the WordPress platform at that time. Ben did the graphic design,and I wrote my first WordPress plugin.
